尿酸干预对老年高血压合并糖尿病患者缺血性脑卒中的影响

Effects of uric acid intervention on the incidence of ischemic stroke in the elderly patients with hypertension complicated with diabetes mellitus

目的研究尿酸干预对老年高血压合并糖尿病患者缺血性脑卒中的影响。方法老年高血压合并糖尿病的高尿酸血症患者100例,随机分为尿酸干预组和对照组,对照组给予控制血压、血糖的常规药物治疗及低嘌呤饮食,尿酸干预组在常规药物治疗及低嘌呤饮食的基础上给予苯溴马隆治疗。对比研究2组干预前、干预后3月、6月及1年血尿酸、空腹血糖、糖化血红蛋白、动态血压,随访2组缺血性脑卒中发生例次。结果 3月、6月及1年后干预组血尿酸及动态血压明显低于干预前(P<0.01);干预组血尿酸3月、6月及1年后低于对照组(P<0.01),动态血压6月及1年后低于对照组(P<0.05);2组间及干预前后空腹血糖、糖化血红蛋白比较差异无统计学意义;1年后干预组缺血性脑卒中发生例次及比例低于对照组(P<0.05)。结论尿酸干预能降低高尿酸血症的老年高血压合并糖尿病患者缺血性脑卒中发生风险。

Objective To study the effects of uric acid intervention on the incidence of ischemic stroke in the elderly patients with hypertension complicated with diabetes mellitus. Methods 100 patients with hypertension complicated with diabetes mellitus presenting with hyperuricemia were randomly divided into intervention group and control group. The control group received anti-hypertension and anti-diabetes mellitus treament and low purine diet. The intervention group received benzbromarone on the basis of anti-hypertension and anti-diabetes mellitus treament and low purine diet. The levels of serum uric acid,fasting blood glucose,glycosylated hemoglobin,blood pressure were detected and analyzed before and 3 months,6 months,and 1 year after intervention. The incidence rate of ischemic stroke was followed up. Results After intervention,the serum uric acid and dynamic blood pressure were significantly decreased (P < 0. 01) . The serum uric acid was lower in the intervention group than that in the control group 3 months,6 months,and 1 year after intervention (P < 0. 01) ,and the dynamic blood was lower in the intervention group than that in the control group 6 months,and 1 year after intervention (P < 0. 05) . The incidence rate of ischemic stroke in the intervention group was lower than that in the control group after 1 year (P < 0. 05) . Conclusions Uric acid intervention can reduce the risk of ischemic stroke in patients with hypertension complicated with diabetes mellitus presenting with hyperuricemia.
