高频超声、乳腺X线检查及高频超声结合乳腺血氧功能成像在乳腺肿块诊断中的价值 被引量:8

摘要 目的比较高频超声、乳腺X线检查及高频超声结合乳腺血氧功能成像在诊断乳腺肿块中的价值。方法回顾性分析高频超声、乳腺X线检查及高频超声结合乳腺血氧功能成像在110例乳腺肿块中的诊断结果,分别比较其对乳腺癌、乳腺纤维腺瘤、乳腺脓肿及乳腺囊肿或囊性病变的诊断灵敏度和特异度,并绘制受试者工作特征(ROC)曲线,计算曲线下面积,同时比较3种检查方法的准确率。结果高频超声、乳腺X线检查及高频超声结合乳腺血氧功能成像对乳腺癌的灵敏度分别为80.00%、70.00%、80.00%;特异度分别为94.94%、98.73%、97.47%。对于乳腺纤维腺瘤的诊断,乳腺X线检查的ROC曲线下面积仅为0.400,明显小于高频超声和高频超声结合乳腺血氧功能成像(均为0.669);乳腺X线检查对乳腺脓肿的诊断准确率也较低,ROC曲线下面积小于高频超声和高频超声结合乳腺血氧成像的ROC曲线下面积(分别为0.641和0.766),乳腺血氧功能成像可通过提示肿块内部血红蛋白浓度来提高临床诊断非急性期乳腺脓肿的准确率。结论高频超声、乳腺X线检查及高频超声结合乳腺血氧功能成像是临床诊断乳腺癌的有效手段之一,可有效提高高频超声对临床症状不明显的乳腺脓肿的诊断准确率。 Objective To compare the diagnostic value of high frequency ultrasound,mammography and high frequency ultrasound combined with breast blood-oxygen function imaging system in diagnosis of breast masses.Methods The images of 110 female patients with breast masses by high frequency ultrasound,mammography and high frequency ultrasound combined with breast blood-oxygen function imaging system were reviewed and the sensitivities and specificities of the three methods were compared.The receiver operating characteristic(ROC) curve was made to calculate the area under the curve to evaluate the accuracy of the three methods in the diagnosis of breast masses.Results The sensitivities of the diagnosis of breast cancer by high frequency ultrasound,mammography and high frequency ultrasound combined with breast blood-oxygen function imaging system were 80.00%,70.00% and 80.00%,respectively.The specificities of the diagnosis of breast cancer by them were 94.94%,98.73% and 97.47%,respectively.The area under ROC for mammography in the diagnosis of breast fibroadenoma was 0.400,smaller than high frequency ultrasound and high frequency ultrasound combined with breast blood-oxygen function imaging system(both 0.669).The accuracy of the diagnosis of mammography in mammary abscess was low,and the area under ROC was smaller than high frequency ultrasound and high frequency ultrasound combined with breast blood-oxygen function imaging system(0.641 and 0.766,respectively).Blood-oxygen function imaging system could detect hemoglobin concentration in the masses to improve the accuracy of the diagnosis of mammary abscess.Conclusion High frequency ultrasound,mammography and high frequency ultrasound combined with breast blood-oxygen function imaging system are the highly effective diagnostic tools for diagnosis of breast cancer.High frequency ultrasound combined with breast blood-oxygen function imaging system can help high frequency ultrasound to improve the diagnosis accuracy of mammary abscess.
