上海浦东潍坊社区45例老年贫血患者的临床分析

Clinical Feature Analysis of 45 Cases of Senile Anemia in Weifang Community, Shanghai

目的探讨上海市潍坊社区老年贫血患者的临床特点及贫血原因。方法对2010年5月～2011年5月在潍坊社区卫生服务中心门诊就诊及住院的65岁以上的45例贫血患者进行回顾性分析及临床随诊。结果 45例患者中,贫血的首要原因为缺铁性贫血,共18例(40%);其次为双向性贫血(缺铁性贫血合并巨幼红细胞性贫血)12例(占26.7%)。原发诱因:胃或十二指肠溃疡及慢性胃炎15例,消化道肿瘤所致的消化道慢性失血7例,膳食结构失调6例。结论老年人易患贫血,于社区医疗机构就诊的患者,起病多隐匿,病因复杂多样,其中继发性贫血较多,应尽早明确病因,以便及时确诊慢性疾病及对症治疗。

Objective To understand the clinical features and causes of anemia among elderly patients in Shanghai Weifang community. Methods Among the anemia outpatients and inpatients over the age of 65 in community health service centers during May 2010 to May 2011, 45 cases of anemia patients were reviewed retrospectively and clinical follow-up was conducted. Results In the 45 cases of elderly patients with anemia, the leading cause for senile anemia is iron deficiency anemia with a total of 18 cases, accounting for 40%; followed by the two-way anemia (iron deficiency anemia combined with megaloblastic anemia) with 12 cases, accounting for 26.7%. 15 patients suffered from gastric or duodenal ulcer and chronic gastritis, 7 patients from chronic gastrointestinal blood loss caused by the tumor, and 6 patients from imbalance of dietary structure. Conclusion The elderly are susceptible to anemia. Patients for treatment in the community medical institution suffer anemia for many complex and diverse causes, with secondary anemia as the frequent one. It is essential to identify the causes clearly as soon as possible for the timely diagnosis of chronic diseases and symptomatic treatment. 【
