2种降糖方案对老年糖尿病患者的疗效及生存质量的评价

Evaluation of the therapeautic effect and the quality of life between two kinds of hypoglycemic scheme in elderly patients with type 2 diabetes mellitus

目的评价2种降糖治疗方案对老年2型糖尿病患者的疗效、并发症发生率以及生存质量的影响。方法入选年龄≥65岁的2型糖尿病患者126例,随机分为2组,分别使用甘精胰岛素和预混胰岛素联合口服降糖药物,随访1年后,观察血糖及糖化血红蛋白指标及并发症发生情况,并借助问卷调查的方法从生理功能、社会功能、心理状态等方面评价患者生存质量改善情况。结果治疗后2组血糖均达标,但甘精胰岛素组较预混胰岛素组达标早,并发症发生率较预混胰岛素组低,治疗前生存质量综合评分为(69.74±6.09)分,治疗后为(74.40±4.58)分,较预混胰岛素组高(P<0.05或P<0.01)。结论甘精胰岛素联合口服降糖药物是一种适合老年2型糖尿病患者的治疗方案,能降低并发症发生率,患者依从性好,生存质量改善。

Objective To evaluate the therapeutic effect,safety and the quality of life between two kinds of hypoglycemic scheme.Methods 126 elderly patients who were older than 65 years with type 2 diabetes mellitus were selected and randomly divided into two groups: glargine combined with oral drugs group and pre-mixed insulin combinded with oral drugs group.After one year follow-up,the change of blood glucose and glycosylated hemoglobin,the incidence of hypoglycemia cardial and cerebral vascular attack were recorded and compared.At the same time,the patients' quality of life including physiological function,social function,psychological status and other aspects were observed through the questionnaire.Results The level of blood glucose reached the standard in both two groups after the treatment.However,the time of blood glucose reaching the standard in glargine insulin group was shorter than that in premixed insulin group.The incidence of complication in largine insulin group was lower and the score of quality of life was better than that in premixed insulin group(P < 0.05 or P < 0.01) Conclusions The glargine insulin combined with oral drugs is a better prescription not only with efficiency but also safety for the elderly patients with type 2 diabetes mellitus.
