改良Child术式在胰十二指肠切除术的应用

Clinical application of modified child pancreaticojejunostomy on prevention of pancreaticfistula after pancreatoduodenectomy

目的 总结改良Child术式预防胰十二指肠切除胆胰瘘。方法回顾分析24例胰十二指肠切除术,采用改良child方式重建消化道(Child术式重建加胃空肠Roux–en-Y吻合)。结果 24例术后恢复良好,黄疸明显消退,仅发生胆胰瘘1例。结论 改良Child术式在预防胰十二指肠切除胆胰瘘不失为一有效方法。

Objective? To?explore?the?ways?how?to?decrease?the?morbidity?and?mortality?of?pancreaticoduo?denectomy?(PD).?Methods? In?24?cases?of?PD,?a?free?vascuIarized?jejunal?loop?was?used?to?perform?single?loop?Roux-Y?anastomosis?to?reconstruct?the?digestive?tract??Results? The?morality?rate?was?zero?and?there?were?no?cases?of?leakage?at?the?pancraticojejunostomy?and?choledocojejunostomy.?There?were?no?cases?of?biler?reflux?gastric?disease,?retrograde?infection,?anastomostic?ulcer?of?gastrojejunostomy,?and?dumping?syndrome.?Conclusion? This?surgical?procedure?can?effectively?reduce?the?morbidity?and?the?mortality?of?PD.
