心肌梗死心脏康复的循证医学证据 被引量:6

摘要 冠心病是目前人类死亡的首要原因之一,其中急性心肌梗死(AMI)是其最严重的临床类型。急性ST段抬高心肌梗死诊疗指南推荐的标准化治疗[再灌注治疗(静脉溶栓或经皮冠状动脉介入治疗)、抗凝及抗血小板、血管紧张素转换酶抑制剂或血管紧张素Ⅱ受体拮抗剂、?-受体阻滞剂和他汀类等]可显著降低AMI患者的病死率,改善临床结果,但AMI患者主要心脏不良事件的发生率仍较高。循证证据表明以运动训练为基础的心脏康复可显著改善心肌梗死(MI)患者的临床结果[1-4],目前的AMI诊疗指南推荐为I/B级[5,6],建议对稳定的AMI(无再发心肌缺血、心力衰竭或严重心律失常)患者卧床不应超过12~24小时,及早对其进行心脏康复,将其作为治疗AMI患者的一颗魔弹提升到新的高度。本文仅对AMI患者进行心脏康复的循证医学依据作一介绍。
