上海市某私营骨科专科医院伤害病例分析

Analysis on Cases of Injury Surveillance System ina Private Orthopedics Hospital in Shanghai

目的 通过分析某私营骨科医院伤害病例流行病学特征,探讨私营专科医院在伤害监测系统中的地位和作用,为完善伤害监测体系提供依据.方法 以上海市长宁区某骨科医院2012年伤害病例为对象,分析人口学特征、伤害特征、受伤原因、伤后处理及疾病负担.结果 共计监测到伤害病例874例,平均年龄34.3岁,男女性别比2.8∶1.就诊病例中70.8％为非上海户籍人口.主要的受伤类型为跌落(52.4％),其次为钝器伤(23.8％)和交通伤害病例(7.7％).43.8％的病例因骨折就诊该医疗机构,其次是开放性伤害(20.6％)和扭伤(18.6％).54.3％的病例为中等程度伤害,重伤15.7％.35.7％的伤害发生在工业和建筑区域.所有病例受伤后平均需要住院7天,休息30天,平均医药费用为2 000元.结论 专科医院在病例人口学特征和伤害特征等方面不同于社区医疗机构和综合性大医院,是伤害监测系统的有力补充.建议在此监测基础上开展针对性预防措施.

Objective To analyze the epidemiological characteristics of casescollected through injury survelilancesystem in a private orthopedics hospital,and to explore the private hospitals in the position and role of injury surveillance system.Method a orthopaedic hospital damage cases in 2012 in Shanghai changning district as the object, analyzing the characteristic of the demographic characteristics, damage, injury causes, injury post-processing and burden of disease.Results of monitoring the damage cases 874 cases, with an average age of 34.3 years old, male and female sex ratio of 2.8:1. The clinic cases 70.8% of the Shanghai population. The main types of injuries to fall (52.4%), followed by blunt injury (23.8%) and traffic injuries cases (7.7%). 43.8% of the cases of the medical institutions because of fracture clinic, followed by open injuries (20.6%) and strain (18.6%). 54.3% of cases of moderate damage, serious injury by 15.7% 35.7% of the damage occurred in the industrial and building area. All cases were injured after 7 days, on average, need to be hospitalized, rest for 30 days, the average medical expenses is RMB 2, 000. Conclusion the specialized subject hospital cases in demographic characteristics and damage characteristics of different on community medical institutions and comprehensive hospitals, and is a powerful supplement of injury surveillance system. Suggestions based on the monitoring to carry out the corresponding preventive measures.
