摘要
目的:探讨腹腔镜技术在泌尿外科重建手术中的应用.方法:总结2003~2012年开展的757例腹腔镜泌尿外科重建手术的应用情况,技术要点.包括:肾盂输尿管成形术96例,肾脏部分切除术258例,腔静脉后输尿管成形术10例,肾盂输尿管切开取石术133例,重复肾切除16例,膀胱部分切除术6例,膀胱根治性切除术131例,前列腺癌根治术142例.结果:手术一次成功率94.2%,手术时间35~480 min,平均(100±33)min,出血量30~2 500 ml,平均(180±39)ml,术后住院时间3~15 d.结论:泌尿外科腹腔镜重建手术难度相对较大,高超的缝合、打结技术会使手术时间大大缩短.术后需长期随访功能改善情况.
Objective:To evaluate the operative technique and the clinical efficacy of laparoscopic technology in urological reconstructive surgery.Methods:792patients were treated with laparoscopic surgery from 2003to 2012,including 96cases of pyeloureteroplasty,258cases of partial nephrectomy,10cases of retrocaval ureter,16cases of nephrectomy of duplicated kidney,133cases of ureterolithotomy,6cases of partial cystotomy,131cases of radical cystectomy and 142cases of radical prostatectomy.Results:The first time success rate of surgery was 94.2%.The operation time ranged f rom 35to 480minutes,mean(100±33)min and the blood loss was 30-2 500ml,mean(180 ±39)ml.The hospital stay was 3to 15days.Conclusions:laparoscopic urological reconstructive surgery is more difficult.Superb stitching,knotting techniques will greatly shorten the operation time.The patiets need long-term follow up after operation.
