支气管肺发育不良早产儿潮气呼吸肺功能的检测 被引量:7

Determination of tidal pulmonary function in premature neonates with bronchopulmonary dysplasia

摘要 目的了解支气管肺发育不良(BPD)早产儿肺功能的特点,为疾病的诊断、治疗提供依据。方法研究对象为60例体重<1.7 kg,胎龄<34周的早产儿,其中BPD组和对照组各30例,在体重>1.8 kg以上、出院时进行潮气呼吸肺功能测定,比较两组差别。结果BPD组在达峰容积比VPEF/VE(23.96±6.13)%和达峰时间比TPEF/TE(21.03±7.20)%较对照组明显减低(P<0.05),潮气呼吸呼气峰流速(PTEF)BPD组较对照组稍高(P<0.05)。其他肺功能指标均与对照组差异无显著性。结论支气管肺发育不良早产儿的潮气肺功能变化提示阻塞性通气障碍,与BPD的严重程度有关。对于BPD的早产儿出院后动态肺功能测定有助于早期发现后期患呼吸系统疾病的风险。 Objectives To investigate the pulmonary function change in premature with hronchopulmonary dysplasia(BPD).Methods According to the diagnostic criteria of bronchopulmonary dysplasia,sixty premature neonates horn<34 weeks of gestation were divded into BPD group(n=30) and control group(n=30).Their tidal lung function was detected when they were ready to he discharged from hospital and body weight>1.8kg.The difference between two groups was compared.Results VPEFNE(23.96±16.13)%and TPEF/TE(21.03±7.20)%in BPD group were lower than those in control group(P<0.05),indicating the obstructive ventilatory defect.PTEF of BPD group was higher than that of control group.In other parameters of tidal lung function,there were,no differnces between the two groups.Conclusion The pulmonary function change of premature neonates with hronchopulmonary dysplasia indicated the obstructive ventilatory defect.The changes reflects severity of BPD.Longitudinal lung function testing of preterm infants after discharge from hospital may help to early predict the risk of later respiratory problems.
