手足口病患儿外周血淋巴细胞亚群变化的临床观察

The changes in peripheral blood lymphocyte subsets in children with hand,foot and mouth disease

目的观察手足口病患儿外周血T淋巴细胞亚群、B淋巴细胞和NK细胞的变化。方法根据病情将50例手足口病患儿分为普通组30例、重症组20例。以I0例健康儿童作为对照组,采用流式细胞仪检测患儿外周血T淋巴细胞亚群,B淋巴细胞和NK细胞。结果手足口病患儿外周血CD3^+细胞、CD4^+细胞和CD8^+细胞的表达:重症组、普通组及对照组数值依次升高,差异均有显著性(P均<0.05);各组之间Th/T,细胞比值的比较,差异均无显著性(P均>0.05);外周血B淋巴细胞((CD19^+)和NK细胞((CD16^+CD56^+)的表达:重症组、普通组及对照组依次降低.差异均有IV著性(P均<0.05)。结论手足口病患儿存在免疫系统功能紊乱。随着病情严重程度加重,T淋巴细胞、NK细胞和B淋巴细胞表达增加,提示细胞免疫功能抑制程度加重和体液免疫反应增强。

Objectives To observe the level of lymphocyte subsets in peripheral blood in children with hand,foot and mouth disease(HFMD).Methods Fifty cases of children with HFMD were divided into mild group and severe group under the condition.Ten healthy children as control group were enrolled in the study.The percentage of lymphocyte subsets,including CD3^+ cells,CD4^+ cells(Th),CI)8^+cells(Ts),CD19^+ cells and CD16^+56^+cells in anticoagulative whole blood of children from the three groups were determined by fluorescence flow cytometry.Results Among three groups,there were significant differences(P<0.05) in the percentage of CD3^+cells,CD4^+ cells(Th),CD8^+cells(Ts),CD19^+cells and CD16^+CD56^+cells.The percentage of CD3^+cells,CD4^+ cells(Th),CD8^+cells(Ts) in severe,mild and control groups was gradually increased in the order.The percentage of CD19^+cells and CDl6^+CD56^+cells in severe,mild and control groups was gradually decreased in the order.There were no significant differences in the Th / Ts cell ratio(P> 0.05) among the three groups.Conclusion There is an immune system dysfunction in the HFMD children.As the severity of disease is aggravating,T lymphocytes,NK cells and B lymphocytes increase their expressions.It is suggested that cellular immune function is inhibited and humoral immune response increased.
