Treatment of femoral trochanteric fracture with dynamic hip screw

目的 探讨动力髋螺钉对股骨粗隆部骨折的治疗效果及其并发症的防治。 方法 回顾性总结分析我科 1996～ 2 0 0 1年采用动力髋螺钉治疗股骨粗隆部骨折 75例的效果及术后并发症情况。 结果 按莫子丹等[1] 的术后评定标准 ,优 6 0例 ,占 80 % ;良 13例 ,占 17 3% ;差 2例 ,占 2 7%。 75例中 7例出现术后并发症 ,包括下肢旋转畸形 3例 ,髋内翻 2例 ,内固定失败 2例。 结论 动力髋螺钉对各种类型的股骨粗隆部骨折均能坚强固定 ,有利于病人的早期功能锻炼及骨折愈合。其造成并发症的原因主要有手术时机延误致手术难度增加 ,手术操作不规范 。

Objective To discuss the treatment effect and preventive measures to the complications for the treatment of femoral trochanteric fracture with dynamic hip screw (DHS). Methods Seven complications occurred during the 75 consecutive cases of femoral torchanteric fracture treated in 1996～2001 were reviewed, and the causes of complications were analyzed Results The complications included 3 lower extremity rotatory deformity, 2 deformity of hip, and 2 failure in fixation Conclusions DHS has strong capacity to fix various femoral trochanteric fracture and benefits of early functional exercise and healing to the patients. The main causes of complications may be as follows: loss of the opportunity to timely operation, lack of standard technique, and the unsuitable indications for operative treatment
