腹腔镜手术治疗巨大输尿管纤维上皮息肉合并输尿管套叠

Laparoscopic Management of Intussusception of the Ureter Caused by a Large Fibroepithelial Polyp

目的探讨巨大输尿管纤维上皮息肉合并输尿管套叠的腹腔镜手术的可行性及疗效。方法 1例左输尿管下段巨大输尿管纤维上皮息肉合并输尿管套叠应用腹腔镜手术治疗,分析治疗过程,随访疗效。结果腹腔镜手术中松解套叠段输尿管,切除输尿管息肉及息肉蒂部附着段输尿管,放置输尿管支架管,端端吻合。术后随访2年,息肉无复发,肾盂及输尿管形态正常。结论腹腔镜手术是治疗较大输尿管息肉的可行方法。如输尿管息肉合并输尿管套叠,腹腔镜手术应为首选。术前明确诊断至关重要。

Objective To investigate the laparoscopic management of intussusception of the ureter caused by a large ureteral fibroepithelial polyp.Methods A case of intussusception caused by a large ureteral fibroepithelial polyp in the lower part of the left ureter was treated with laparoscopic approach.Results Unfolding of the intussusception and resection of the the segment of ureter with the stalk attached to the body of the polyp were successfully performed laparoscopicly.Anastomosis of the ureter was performed after a double-J stent was placed.The double-J stent was removed 6 weeks postoperatively.A B-ultrasound revealed a normal urinary system 2 years after the operation,no polyp recurred.Conclusions Laparoscopic approach is feasible to treat a large ureteral polyp.For large polyp of ureter complicated with intussusception,a laparoscopic approach is preferred.It's paramount to identify the diagnosis before treatment.
