209247 Analysis of mortality risk factors in children complicated acute renal dysfunction after congenital cardiac surgery/Huang Jihong(Dept Thorac Cardiovasc Srug,Shanghai Children’s Med Center,Shanghai Jiaotong Univ Med Col,Shanghai 200127)…∥Chin J Thorac Cardiovasc Surg.-2009,25(1).-32~34Objective Peritoneal dialysis offers one of effective methods for treating acute renal dyfunction after congenital cardiac surgery,however the mortality of these patients are still relatively high.The aim of this study was trying to predict the risk factors of mortality risk at initiation of peritoneal dialysis using clinical and laboratory data.Methods From Jun.2006 to Jan.2007,1312 patients with congenital heart disease were treated with cardiac surgery.Twenty seven patients accepted dialysis due to acute renal dysfunction.We analyzed retrospectively the clinical and experimental data obtained befroe starting peritoneal dialysis.Results In 27 children with peritoneal dialysis,there were 13 nonsurvivors(48.1%),14 survivors,there was a significant difference between survivors and nonsurvivors with regard to the viriables parameters such as cardiac troponin I(cTnI),pH,PIP,colloid osmotic pressure(COP),base excess(BE),lactate(Lact),arterial-venous oxygen saturation difference(ΔSO2),and inotropic score(InS).The stepwise regression analysis indicates a multivariate influence of cTnI(P=0.01),pH(P=0.04),PIP(P=0.02),COP(P=0.05) on mortality.Conclusion The variables of cTnI,pH,PIP and COP can help to predict mortality as risk factors at the initiation of peritoneal dialysis.8 refs.
