脐旁皮瓣带腹直肌前鞘游离移植修复足跟踝后皮肤与跟腱缺损

FREE GRAFTING OF PARUMBILICAL SKIN FLAPS AND VAGINA MUSCULI RECTI ABDOMINIS TO REPAIR THE SKIN DEFFECTS OF HEELS AND CALCANEUS POSTERIOR ANKLES AND THE DEFFECTS OF TENDO CALCANEUS

目的 探讨脐旁皮瓣带腹直肌前鞘一期修复足跟及踝后皮肤与跟腱缺损的效果。方法 对 6例足跟、踝后皮肤软组织并跟腱缺损的病例采用带腹直肌前鞘的脐旁皮瓣一期修复 ,皮瓣面积最大 12cm× 8cm ,最小 8cm× 4cm。跟腱缺损长度最长 8cm ,最短 4cm ,平均 (6± 0 .3)cm。随访 8～ 2 8个月。结果 皮瓣全部成活 ,皮瓣质地优良 ,外观及功能满意 ;术后半年复查 ,步态正常 ,踝伸屈动作有力 ,活动范围正常。结论 脐旁皮瓣带腹直肌前鞘为一可靠复合组织皮瓣。腹壁下血管解剖固定 ,该皮瓣切取方便 ,皮瓣及前鞘切取面积大 ,可同时修复皮肤软组织及跟腱缺损 ,是一种良好方法。

Objective To explore the curative effects of free grafting of parumbilical skin flaps and vagina musculi recti abdominis to repair the skin deffects of heels and posterior ankles and the deffects of tendo calcaneus.Methods 6 cases of defects of heel skin,posterior ankle skin and tendo calcaneus were treated through the grafting of parumbilical skin flaps and vagina musculi recti abdominis.The largest and the smallest areas of transplanted skin flaps were 12cm×8cm and 8cm×4cm respectively.The longest and the shortest tendo calcaneus with defects were of 8cm and 4cm respectively,and their average length was (6±0.3).Results The 8～28 months of follow-up surveys showed that all the transplanted skin flaps survived well.Their functions and outlooks recovered satisfactorily.Conclusion This kind of skin grafting operates easily and have good curative effects.
