窒息新生儿甲状腺功能变化研究 被引量:1

摘要 目的 研究新生儿窒息后甲状腺功能的变化。方法 对 6 3例窒息新生儿、30例足月正常新生儿及 6 0例正常儿童检测T3 、T4、FT3 、FT4及TSH ,并复查窒息组异常者 ,直到正常为止。结果 与对照组相比 ,轻度窒息组T3 、FT3 降低 (P <0 .0 5 ) ,T4、FT4、TSH无显著变化 (P >0 .0 5 ) ;重度窒息组T3 、FT3 、T4、FT4明显降低 (P <0 .0 1) ,TSH升高 (P <0 .0 5 )。 4 8例甲状腺功能异常者中 32例在出生 13d内恢复正常 ,其水平与儿童正常值无明显差异 (P >0 .0 5 )。出生 2 5d内甲状腺功能全部恢复正常。结论 新生儿窒息可致甲状腺功能一过性减低 ,且其减低与窒息程度呈正相关。 Objective To study the thyroid function changes in asphyxial newborns.Methods T 3,T 4,FT 3,FT 4 and TSH (thyroid stimulating hormone) were measured in 63 cases of asphyxial newborns and 30 cases of normal full-term newborns and 60 normal children.Asphyxial newborns were followed up until the results were restored to the normal level.The results were analyzed and compared between the asphyxia group and the normal control group.Results The levels of T 3 and FT 3 decreased significantly (P< 0.05),but the levels of T 4,FT 4 and TSH changed insignificantly in the group of mild asphyxia.The levels of T 3,FT 3,T 4 and FT 4 decreased significantly(P<0.01),but the level of TSH increased significantly(P<0.01) in the group of severe asphyxia.48 cases (67%) of patients with abnormal thyroid functions recovered in 13 days with no significant differences from normal controls(P> 0.05).All patients recovered in 25 days.Conclusion Asphyxia can cause hypothyroidism and the reducing rate of the disease is in positive proportion to the degree of asphyxia.
