艾迪注射液联合放疗治疗食管癌临床疗效的系统评价

Aidi Combined with Radiotherapy Versus Radiotherapy Monotherapy for Esophageal Carcinoma: A Systematic Review

目的:系统评价艾迪注射液联合局部放疗与单纯放疗比较治疗食管癌的临床疗效。方法:采用Cochrane系统评价的方法,检索PubMed、中文科技期刊全文数据库、中国期刊全文数据库、万方数据库、Cochrane图书馆临床对照试验库等,检索时间均为各数据库建库至2013年10月,纳入艾迪注射液联合放疗治疗食管癌的随机对照研究,由2名评价者独立评价并交叉核对纳入研究质量,对纳入的研究采用Rev Man 5.0软件进行Meta分析。结果:共纳入8项研究,均为低质量研究。系统评价分析结果显示,与单纯放疗方案比较,艾迪注射液与放疗联合可有效改善患者近期疗效[OR=1.88,95%CI(1.23,2.85),P=0.003,提高患者生存质量[OR=1.98,95%CI(1.67,2.34),P=0.002],缓解放疗后放射性食管炎的发生[OR=0.41,95%CI(0.26,0.67)P=0.0003],减少血液学毒性[OR=0.25,95%CI(0.16,0.40),P<0.00001]。结论:艾迪注射液联合放疗对晚期食管癌有很好的辅助治疗作用,能够改善近期疗效,提高生存质量。

Objective:To evaluate the Clinical efficacy of Aidi injection combined with radiotherapy vs .radiotherapy monotherapy alone for esophageal carcinoma .Methods:We searched the Cochrane Library ,PubMed,VIP,Wanfang Data-base and CNKI by computer .The retrieval time was from the building of the datebase to October 2013 .Randomized con-trolled trials ( RCTs) about Aidi injection combined with radiotherapy for esophageal carcinoma were included .The quality of included trials were evaluated by two reviewers independently .Meta-analysis was carried out with RevMan 5.2 software. Results:8 RCTs were included in the meta-analysis,but the quality of reports was poor .Results of meta-analysis showed that Aidi injection combined with radiotherapy could effectively improve short -term efficacy[OR=1.88,95%CI(1.23,2.85),P<0.01],improve patients''quality of life [OR=1.98,95%CI (1.67,2.34),P<0.01],reduce the incidence of ra-diation esophagit [OR=0.41,95%CI (0.26,0.67),P<0.01]and bone marrow suppression[OR=0.25,95%CI(0.16, 0.40),P<0.01].Conclusion:Aidi injection combined with radiotherapy may be an effective regimen in the treatment of esophageal Carcinoma by improving patients short-term efficacy and life quality;reducing the the incidence of radiation esophagit and bone marrow suppression .
