乳腺癌MRI周围血管征与诺丁汉预后指数的相关性 被引量:4

摘要 目的:探讨乳腺癌动态增强MRI( dynamic enhancement MRI ,DCE-MRI)经最大强度投影( maximum intensi-ty projection,MIP)技术处理后所显示的肿瘤周围血管征( adjacent vessel sign ,AVS)与诺丁汉预后指数( nottingham prog-nostic index,NPI)的相关性。方法经手术证实的乳腺癌51例,采用Spearman等级相关统计法,计算51例患者MRI 3D-MIP图上所显示的肿瘤AVS表现与NPI评分等预后指标的相关性。结果 NPI评分<3.4分20例,3.4~5.4分14例,>5.4分17例。随着NPI分值的升高,肿瘤的AVS分级亦随之增加,两者具有相关性(γ=0.601,P<0.001)。 AVS表现与乳腺癌大小及淋巴结状态有显著相关性,但与肿瘤病理学分级无关(γ=0.139,P>0.05)。肿瘤周围血管征与乳腺癌HER-2阳性表达有一定的相关性(0.05<P<0.1)。结论乳腺癌的周围血管征对预测肿瘤NPI分值大小、评估乳腺癌预后以及了解其生物学行为具有重要的意义。 Objective To evaluate the relationship between adjacent vessel sign ( AVS ) of malignant breast lesions , which was displayed by the maximum intensity projection ( MIP) of dynamic enhancement MRI ,and the Nottingham prognostic in-dex(NPI).Methods 51 malignant breast lesions which confirmed by surgical pathology were analyzed retrospectively .The corre-lation between tumors ’ AVS on the 3D-MIP picture and NPI score were analyzed by Spearman χ2 test.Results The number of cases with NPI <3.4,3.4~5.4,>5.4 were 20,14 and 17,respectively.With the increase of NPI,the number of AVS increased (γ=0.601,P<0.001).AVS was closely correlated with tumor size and lymph node status but not related to pathological grading (γ=0.139,P>0.05),and AVS and the positive expression of HER-2 had correlation.Conclusion AVS in the breast cancer plays a significant role in assessing the score of NPI ,evaluating prognosis of breast cancer and understanding biological behavior of the tumor.
