乳腺癌保乳术后调强放疗与常规放疗参数的比较 被引量:29

摘要 目的探讨乳腺癌保乳术后调强放疗与常规放疗计划在靶区剂量学及正常危及器官保护的差异。方法 10例乳腺癌保乳术后患者(4例左侧乳腺癌,6例右侧乳腺癌)均给予50 Gy/25 f放疗,每例均设计调强计划(IMRT)和常规计划(CRT)。比较两者在靶区剂量分布(PTV剂量体积及均匀性指数)和正常危及器官受量(肺脏、对侧乳腺和心脏)差异。结果调强放疗和常规放疗计划中PTV,其中的V95%和V107%有显著性差异(P<0.01,P<0.05)。调强放疗中V110%和V115%低于常规放疗,但无统计学差异(P>0.05)。调强放疗的均匀性指数明显优于常规放疗(P<0.001)。在肺脏受量上,调强放疗与常规放疗计划在V10、V20、V30和V40均有显著性差异(P<0.001)。对侧乳腺受量最大值,调强放疗与常规放疗计划有显著性差异(P<0.05)。左侧乳腺癌患者心脏剂量体积V5和V30两者未见显著差异。结论对于乳腺癌保乳术后患者,调强放疗较常规放疗计划靶区分布更优,均匀性更高,且能够明显降低肺脏和对侧乳腺受照量。
