中西医结合治疗对老年2型糖尿病患者微血管病变的影响

Effect of combination of Chinese traditional and Western medicine on microvascular disease in elderly type 2 diabetes

目的观察中西医结合治疗老年2型糖尿病患者微血管病变的影响。方法老年2型糖尿病(T2DM)患者100例,随机分为对照组和治疗组,每组50例,对照组采用常规西药治疗方法;治疗组在对照组基础上同时加用自拟中药方治疗,两组疗程均为2个月。观察两组治疗前后患者临床疗效、神经传导速度(NCV)、神经病变积分(MDNS)、全血黏度、血浆黏度、血细胞容积、空腹血糖(FPG)、餐后2 h血糖(2 h PG)、24 h尿蛋白及血脂水平变化。结果治疗组总有效率88.0%明显高于对照组总有效率78%(χ2=4.201,P<0.05);两组治疗后NCV、MDNS的水平与治疗前比较差异(P<0.05);治疗组患者治疗后NCV、MDNS的水平与对照组治疗后比较差异(P<0.05);两组治疗后全血黏度、血浆黏度、血细胞容积的变化与治疗前比较差异(P<0.05);治疗组患者治疗后全血黏度、血浆黏度的水平与对照组治疗后比较差异(P<0.05);两组治疗后FPG、2 h PG、HbA1c、甘油三酯(TG)、总胆固醇(TC)的变化与治疗前比较差异(P<0.05);治疗组患者治疗后FPG、2 h PG、HbA1c、TG、TC的变化与对照组治疗后比较差异均无统计学意义(P>0.05)。结论中西医结合治疗老年T2DM患者及改善糖尿病微血管病变疗效显著。

Objective To observe the clinical effect of integrated traditional Chinese and Western medicine in treatment of elderly patients with type 2 diabetes and diabetic microangiopathy.Methods 100 cases of elderly patients with type 2 diabetes were randomly di-vided into control and treatment groups , 50 cases in each group.Patients in the control group were treated with conventional western medicine treatment, in the treatment group were combined with the prescription treatment.Patients in two groups were treated for 2 months.Clinical efficacy, NCV, MDNS, whole blood viscosity, plasma viscosity, hematocrit, fasting blood glucose (FPG), 2 h postprandial blood glucose (2 h PG), 24 hour urine protein and blood lipid level of patients from two groups were observed before and after treatment .Results Total effectiveness of treatment group was 88%, and was 78%in control group , there was significant difference between two groups (χ2=4.201 , P<0.05 ).There were significant differences in the changes of NCV , MDNS of the two groups before and after treatment ( P<0.05 ) .There were significant differences in the changes of NCV and MDNS between the treatment and control groups after treatment ( P<0.05 ).There were significant differences in the blood viscosity , plasma viscosity , hematocrit of two groups between before and after treatment ( P<0.05 ).There were significant differences in whole blood viscosity and plasma viscosity between the two groups after treatment ( P<0.05 ).There were significant differences in FPG , 2 h PG, HbA1c, TG, TC of the two groups before and after treatment (P<0.05).There were no signif-icant differences in FPG, 2 h PG, HbA1c, FPG,TG, TC between two groups after treatment (P>0.05).Conclusions Effect of combina-tion traditional Chinese and Western medicine on treatment of elderly patients with type 2 diabetes and diabetic microangiopathy is signifi-cant.
