CT血管成像对脑静脉畸形的诊断价值

Diagnostic value of CT angiography in cerebral venous malformation

目的探讨CT血管成像对脑静脉畸形(CVM)的诊断价值。方法采用128层螺旋CT扫描仪对15例脑静脉畸形患者进行CT血管成像,分析其CT表现特点。结果 15例患者均为单发脑静脉畸形,左顶叶、右顶叶、左小脑半球、右小脑半球、桥脑受累分别为6、3、3、2、1例;其中1例合并前颅底硬脑膜动静脉瘘畸形,1例合并颈内动脉颅内段动脉瘤。CT表现为多条髓静脉汇入粗大的引流静脉,典型者呈"水母头"状或"树根"状改变。结论 CT血管成像可显示脑静脉畸形特征和伴发病变,对脑静脉畸形具有较大的诊断价值。

Objective To investigate the diagnostic value of CT angiography(CTA) in cerebral venous malformation(CVM). Methods Fifteen CVM patients underwent 128 slice spiral CT angiography, and their imaging features were analyzed. Results All cases showed solitary CVM, and the lesions were located in left parietal lobe(6 cases), right parietal lobe(3), left cerebellar hemisphere(3), right cerebellar hemisphere(2), and pons(1). In addition, dural arteriovenous fistula of the anterior cranial base or intracranial aneurysm of the internal carotid artery was observed in two cases. CTA imaging showed multiple medullary veins importing into gross draining veins, and the typical characteristics included 'caput medusas'or 'tree root'appearance. Conclusion CTA can reveal the characteristic findings of CVM and associated lesions, which is valuable in the diagnosis of CVM.
