中老年糖尿病眼底病变患者白内障术后的黄斑水肿和视力

Macular edema and visual observation of elderly patients with diabetic retinopathy after cataract surgery

目的探讨中老年糖尿病(DM)眼底病变患者白内障术后的黄斑水肿和视力改变情况。方法选取该院2010年10月至2012年12月诊治的老年DM眼底病变的白内障患者71例(患眼74只)为观察组,选取同期非DM老年眼底病变的白内障患者69例(患眼73只)为对照组,均行白内障手术治疗。比较两组患者血糖水平、视力改变情况、术后渗漏情况、黄斑水肿情况、并发症情况。结果观察组患者空腹血糖、餐后2 h血糖、并发症发生率均明显高于对照组,对照组患者在治疗1、6 w、6个月时矫正视力均明显好于观察组,对照组患者白内障术后渗漏情况明显好于观察组,观察组患者黄斑中心凹厚度明显大于对照组(均P<0.05)。结论中老年DM眼底病变患者白内障术后容易发生黄斑水肿,会明显影响到患者术后视力的恢复状况。

Objective To investigate macular edema and visual acuity changes of elderly patients with diabetic retinopathy after cat- aract surgery.Methods 71 elderly patients with diabetic retinopathy(74 eyes) were selected in hospital from October 2010 to December 2012 as observation group.69 elderly patients with non-diabetic retinopathy(73 eyes) were selected in hospital at same period as control group.Patients were treated with cataract surgery.The levels of blood glucose,visual changes,postoperative leakage,macular edema,compli- cations were compared between two groups.Results The levels of fasting blood glucose,postprandial 2 h blood glucose,complication rate in observation group were significantly higher than those of control group.Corrected visual acuity in control group was significantly better than that of observation group after 1,6 weeks,6 months treatment.Cataract surgery leakage in control group was significantly better than that of observation group.Foveal thickness in observation group was significantly greater than that of control group(P<0.05).Conclusions Eld- erly patients with diabetic retinopathy prone to macular,which will significantly affect recovery of visual acuity after surgery.
