5步运动疗法对COPD机械通气患者的脱机疗效观察

Observation of the curative effect of kinesiotherapy on the patients undergoing mechanical ventilation with off-line and COPD

目的:研究5步运动疗法对慢性阻塞性肺病致呼吸衰竭行有创机械通气患者的脱机疗效。方法:收集2008年2月至2009年12月入住我院综合ICU的行机械通气治疗COPD患者临床资料,在征求患者同意的前提下,按是否在常规治疗的基础上行5步运动疗法分为试验组和对照组,经过相关数据的对比以评价此疗法对患者的疗效。结果:机械通气第15天和第25天,试验组潮气量、呼吸频率、动脉血气PaCO_2指标均优于对照组(P<0.01);机械通气第25天,试验组心率优于对照组(P<0.01);试验组患者呼吸机相关性肺炎(Ventilation Associated Pneumonia,VAP)发生率、机械通气日及ICU住院日均明显少于对照组(P<0.05)。结论:运动疗法对行有创机械通气的COPD患者的脱机有积极作用。

[Objective]To observe the curative effect of kinesiotherapy on the patients undergoing mechanical ventilation with CDPD and off- line.[Method]The experimental group in the conventional treatment carries on gradually the 5 step kinesiotherapy according to the patient condition and the physical strength;Control group was treated with conventional method.[Result] Compared with the control group,the VT,f,PaCO2,HR and the incidence of the VAP were better,and the time of mechanical ventilation and ICU hospitalization were shorter in the experimental group.[Conclusion]The kinesiotherapy may play a positive role in the off- line of the COPD patient undergoing mechanical ventilation.
