摘要
目的探讨储存期间库存血液游离血红蛋白(FHb)水平,提高血液质量和患者用血安全。方法研究对象以周为时间段,将2008年5~9月间157袋库存血液按照血液储存时间分为A、B、C、D、E 5组。A组为储存0~7 d的血液(28袋);B组为储存8~14 d的血液(33袋);C组为储存15~21 d的血液(39袋);D组为储存22~28 d的血液(32袋);E组为储存29~35 d的血液(25袋)。分别对各组库存血液进行游离血红蛋白测定,方法采用分光光度计比色法。结果A、B、C 3组间结果比较,差异无统计学意义(P】0.05);D、E组结果较A、B、C 3组FHb明显升高,差异有统计学意义(P【0.01);E组较D组高,差异有统计学意义(P【0.01)。结论悬浮红细胞上清液中游离血红蛋白随储存期的延长有上升趋势,对于储存期末的血液,特别是最后1周内的血液,在用于临床前应注意密切观察其溶血情况,避免输注过高的游离血红蛋白血液引起急性肾功能衰竭等严重副作用,提高血液输注安全。
Objective To explore the impact of storage time on level of free hemoglobin in red blood cell(RBC) suspension,and to improve the quality and safety of transfused blood.Methods A total of 157 bags of stored RBC suspension were divided into Group A,B,C,D,E according to the storage time.The RBC suspensions in Group A,B,C,D,E were stored for 0-7 d(n=28),8-14 d(n=33),15-21 d(n=39),22-28 d(n=32),and 29-35 d(n=25),respectively.The free hemoglobin level of the RBC suspension was determined with colorimetric-spectrophotometric method.Results There were no significant differences in free hemoglobin level among Group A,B and C(P>0.05).The free hemoglobin levels were significantly higher in Group D and E than in Group A,B and C(P<0.05).Also the free hemoglobin level was significantly higher in Group E than in Group D(P<0.01).Conclusion The level of free hemoglobin in supernatant of RBC suspension increases with increasing storage time.The hemolysis in the blood with longer storage time,especially in those stored for 29-35 days should be noticed before transfusion.The adverse effect such as acute renal failure due to transfusion of hemolyzed blood should be avoid to improve the safety of transfusion.
