辛伐他汀对血液透析患者血脂水平影响的初步研究

The influence of simvastatin on serum lipid level in patients of hemodialysis:A preliminary study.

目的观察维持性血液透析(MHD)患者血清血脂水平及其特点,前瞻性观察辛伐他汀对MHD患者血清总胆固醇(TC)、三酰甘油(TG)、低密度脂蛋白(LDL-C)和高密度脂蛋白胆固醇(HDL-C)浓度的影响。方法测定29例MHD患者血清血脂水平,使用辛伐他汀20mg,每日1次,共8周对其中存在血脂异常的13例患者进行干预治疗,其余16例作为对照组;测定两组研究前后血脂水平变化。结果本组MHD患者血脂异常以TG升高和HDL-C降低为主要特点,使用辛伐他汀治疗8周后,试验组血清TC、TG显著降低,LDL-C下降,HDL-C轻度升高,TG降低幅度高于TC、LDL-C下降幅度。结论本研究显示,每晚口服辛伐他汀20mg 8周能显著降低MHD患者TC、TG水平,TG对辛伐他汀治疗更加敏感,TG降低的幅度或比例超过TC和LDL-C下降的幅度或比例。

Objective To observe the serum lipid profile and its characteristic in a group of maintenance hemodialysis (MDH) patients.And prospectively study the influence of simvastatin on total cholesterol(TC),triglyceride(TG), low-density lipoprotein cholesterol(LDL-C),and high-density lipoprotein cholesterol(HDL-C) of MDH patients.Methods The serum lipid levels of 29 MHD patients were assayed.And those with dyslipidemia were assigned into experiment group(13 cases) and applied with 20mg simvastatin per night for 8 weeks.Others(16 cases) were assigned into control group.Before and after experiment, the serum TC,TG,LDL-C and HDL-C levels were determined.Results The main features of dyslipidemia in the group of MHD patients were elevation of TG and decrease of HDL-C concentration.After 8-week simvastatin intervention, the TC and TG levels in experiment group were significantly lowered,LDL-C decreased and HDL-C rose by a certain margin,though not significantly.The percentage change of TG was remarkably higher than that of TC or LDL-C. Conclusion According to our preliminary study,20mg simvastatin for 8 weeks could significantly lower TC and TG in a group of MHD patients.And serum TG seemed more sensitive to simvastatin treatment and its alteration margin or proportion were larger than that of TG or LDL-C.
