标准大骨瓣减压术治疗重型颅脑损伤患者的临床体会

Clinical experience of standard large trauma craniectomy in treating severe traumatic brain injured patients

目的探讨标准创伤大骨瓣减压术治疗重型颅脑损伤的临床效果。方法对58例重型颅脑损伤患者均采用标准创伤大骨瓣开颅减压术治疗并观察其治疗效果。结果6个月按照GOS分级方法进行预后评估:恢复良好25例,重残13例,持续性植物状态生存9例,死亡11例。结论标准创伤大骨瓣减压术比常规骨瓣减压术可使脑组织得到充分减压,有效保护脑功能,明显改善重型颅脑损伤患者的预后。

Objectiv To explore the efficacy of large standard trauma craniotomy in treating severe traumatic brain injured patients.Methods Standard large trauma craniectomy was performed in 58 cases with severe traumatic brain injured,and the therapeutic efficacy was observed.Results After 6 months follow up,25 cases recovered well,13 cases severely disabled,9 cases was under vegetative state and 11 cases died.Conclusion The standard large trauma craniotomy reduced intracranial pressure effectively and protected the function of brain tissue.The prognosis of severe traumatic brain injured patients could be improved by this technique.
