腮腺腺淋巴瘤的MRI表现(附30例分析)

MRI manifestations of adenolymphoma in parotid gland:a report of 30 cases

目的探讨腮腺腺淋巴瘤的MRI表现,旨在提高对本病的认识。方法回顾性分析经手术病理证实的30例腮腺腺淋巴瘤患者的临床及MRI表现。结果30例腮腺腺淋巴瘤患者中,男25例,女5例,年龄19～82岁(平均56岁),50岁以上21例(70.0%);有吸烟史23例(76.7%)。30例腮腺腺淋巴瘤共发现45个肿瘤(其中单侧单发23例,单侧多发3例,双侧多发4例),其中35个(77.8%)肿瘤位于腮腺后下方。肿瘤一般呈圆形、椭圆形或分叶状软组织肿块,边缘光整;T1WI呈低信号,T2WI及压脂T2WI呈低、高混杂信号,囊变区呈明显长T1、长T2信号,增强后表现为早期轻中度强化。结论腮腺腺淋巴瘤有一定的临床和影像特点;术前MR检查,不但可对肿瘤进行术前准确定位和发现多发病灶,而且有利于肿瘤的诊断、鉴别诊断和治疗方法的选择。

Objective To analyze the MRI appearances of adenolymphoma in parotid gland in order to improve the cognition.Methods Clinical data and MRI findings of 30 patients with adenolymphoma in parotid gland were retrospectively analyzed.Results Among 30 patients,25 were male and 5 were female;aged between 19 and 82(mean age 56).A total of 21(70.0%)were over 50 years old and 23(76.7%)had smoking habit.A total of 45 lesions were found including 23 solitary and 7 multiple lesions.Most adenolymphoma(77.8%)located in the posterior and inferior quadrant.On MRI,lesions were round,oval or lobulated in shape,well-defined,hypointense on T1WI,low or hyterintense on T2WI.Cystic area showed long T1,T2 signal intensities.Most of the lesions showed early mild or moderate enhancement after contrast administration.Conclusion Adenolymphoma in parotid gland has some clinical and imaging characteristics.MR examination before operation can not only well demonstrate the size,number and location of adenolymphoma,but also help for the diagnosis,differentiation and therapeutic decision.
