腋下淋巴结影像参与BI-RADS系统分级的应用价值 被引量:1

摘要 目的探讨腋下淋巴结影像参与BI-RADS系统对乳腺病变分级的应用价值。方法选取乳腺CR片内显示腋下淋巴结影像者78例,将该组CR片由两组医师按双肓法分别进行BI-RADS分级评估,试验组医师评估时将腋下淋巴结影像(形态、密度及大小)列入BI-RADS分级系统中按新标准进行分级评估。对照组医师仍按照第四版乳腺影像报告和数据系统(breast imaging reporting and data system,BI-RADS)进行分级评估(即不将腋下淋巴结影像列入BI-RADS分级系统中)。将两组分级评估结果分别与手术病理结果进行对照研究。结果两组结果相对比,BI-RADS分级为5级的评估例数有统计学差异。结论将腋下淋巴结影像(形态、密度及大小)列入BI-RADS分级系统中进行分级评估,极大地提高了BI-RADS分级为5级的病例中乳腺癌的检出率及与病理结果对照的符合率,使病例分级评估结果与手术病理结果更加接近,从而极大的减少了乳腺癌的误诊及漏诊率,特别是减少了隐匿性乳腺癌的漏诊。
