摘要
目的探讨椎-基动脉系统缺血病人血管结构异常的影像学特征和临床意义。方法 对近4年来发现的椎-基动脉系统缺血病人,包括椎-基动脉供血不足和TIA,进行MRA和DSA检查,并进行综合分析。结果,本组病人中常见的血管结构异常包括起始异常、过度弯曲或迂曲、移位、缩细、狭窄、阻塞和消失。结论血管结构异常是重要的椎-基动脉系统缺血的发病基础。对血管结构异常的分布和分类进行分析,提示椎动脉血管异常是椎-基动脉系统缺血的主要原因,狭窄居所有血管异常之首。MRA和DSA一样对椎-基动脉系统血管异常具有确定的诊断价值。
Objective To investigate and discuss the imaging characteristics and clinical significance of vascular structural abnormalities in patients with vertebrobasilar ischemia. Methods 66 cases with vertebrobasilar ischemia including verte-brobasilar insufficiency and TIA were examined by DSA or MRA within recent 4 years. Results Common structural abnormalities that we found included abnormal origin,excessive bending or meandering,displacement .thinning,stenosis .occlusion and disappearance. Conclusion Vascular structural abnormalities were the important pathogenetic basis of vertebrobasilar ischemia. The distribution and categories of vascular structural abnormalities suggested that vascular abnormities of vertebral artery were the main cause of vertebrobasilar ischemia and stenosis was in the first place among these vascular disorders. MRA as well as DSA had definite diagnostic value for vertebrobasilar vascular abnormalities.
