ET与EP方案治疗晚期非小细胞肺癌的对比研究

Comparative Study of ET and EP Regimen in Advanced Stage Non-small Cell Lung Cancer

目的 对比ET与EP两组方案治疗晚期非小细胞肺癌的疗效和毒性。方法 通过ET和EP两个联合化学治疗方案每21～28天周期全身给药的方法,共收治晚期非小细胞肺癌65例,其中腺癌42例,鳞癌21例,其他2例。初治41例,复治24例。结果 ET和EP组有效率分别为53.3％(16/30)和28.6％(10/35),两组有显著性差异(P<0.05)。毒副作用主要为骨髓抑制,Ⅲ～Ⅳ度白细胞下降率:ET组为50％(15/30);EP组为25.7％(9/35),两组间有显著性差异(P<0.05),;Ⅲ～Ⅳ度血小板下降率:ET组为6.7％(2/30),EP组为5.7％(2/35);Ⅲ～Ⅳ度消化道反应两组发生率分别为23.3％(7/30)和28.6％(10/35),且以置度为主;ET组Ⅲ度过敏反应2例。结论 ET联合方案对晚期非小细胞肺癌有较好疗效,不良反应可以耐受。

Objective To compare the efficacy and toxicity in the treatment of non-small-cell lung cancer (NSCLC) with ET and EP regimen. Methods Through every 21 to 28 days cycle, systemic administration of ET and EP combination chemotherapeutic regimen,a total of 65 cases of advanced stage non-small-cell lung cancer were enrolled. Among them,42 cases were adenocarcinoma,21 spuamous cell carcinoma and 2 others. Chemotherapy-naive patients account for 41 cases and 24 recurrent diseases. Results The response rate were 53. 3%(16/30)for ET regimen and 28. 6%(10/35)for EP regimen respectively. There was a significant difference (P<0. 05) between them. The major toxicity was bone marrow suppression. Grade Ⅲ-Ⅳ leukopenia occurred in 50%(16/30)of the ET group and 25. 7%(9/35)of the EP group. There was a significant difference (P<0. 05)between two groups. Grade Ⅲ-Ⅳ thrombocytopenia was 6. 7%(2/30)in ET group and 5. 7%(2/35)in EP group(P>0. 05).In grade Ⅲ-Ⅳ,nausea and vomiting were observed in 23. 3%(7/30)with ET and 28. 6% (10/35)(P> 0. 05)and mainly occurred with grade Ⅲ toxicity in both groups. Conclusion Taxotrer was effective in the combined treatment of advanced non-small-cell lung cancer. Significant leukopenia occurred but was all tolerated well.
