摘要 目的 射频消融治疗肝脏肿瘤的机制目前尚不明了 ,我们通过本实验观察射频治疗后兔肿瘤模型及临床肝癌病例的分子生物学改变。方法 实验兔 3 2只制成VX2 细胞肝癌模型 ,分为射频组 ( 2 8只 )即术后 0、2、6、12、2 4、48、168h和对照组 ( 4只 ) ,通过HE染色、TUNEL法及二苯胺法观察肿瘤组织及治疗旁中心区组织的凋亡指数改变。结果 射频组肿瘤经射频治疗后发生凝固性坏死及细胞凋亡 ,凝固性坏死边缘区肿瘤组织凋亡指数明显高于对照组 ,其指数术后 0h即升高 ,术后 12h均值达高峰 (P <0 .0 1) ,后逐渐下降 ,但术后 168h仍高于术前组。结论 射频治疗通过诱导高温凝固性坏死区周边肿瘤细胞凋亡 ,进一步扩大了射频治疗的治疗范围 。 Objective To study the molecular biological alterations in rabbit hepatic cancer models and clinical hepatic cancer cases subject to radio-frequency ablation (RFA).Methods The VX-2 tumor cells were implanted in the hepatic left-central lobe of 32 rabbits. All animals were divided into two groups randomly:the RFA group (n=28) and the control group (n=4).The changes in apoptotic index in the tumor tissue and the tissue adjacent to the treatment center were observed by using HE staining,TUNEL and Diphenylamine methods on 0,2,6,12,24,48,168 h after the operation in RFA group, and in the control group.Results Coagulative necrosis and apoptosis occurred in the RFA group after RFA.The apoptotic index of the tumor tissue in the edge of the coagulative necrosis zone was obviously higher than that of the control group.The apoptotic index was increased immediately after operation,peaked at 12 h after operation (P<0.01),then gradually descended,But at 168 h after RFA,it was also high than that of control group.Conclusion RFA treatment can extend its therapeutic zone though inducing the tumor cell apoptosis in the periphery area around the coagulative necrosis zone.Inducing the tumor cell apoptosis is a very important mechanism of RFA to treat malignant tumor.
