伤指浅腱滑行移植修复屈指深腱缺损

Sliding graft of the injuried finger FDS in repairing the FDP defect

目的:报道伤指浅腱滑行移植修复屈指深腱缺损的临床治疗效果。方法:自1992年10月至1996年12月,用该方法治疗患者49例84指,全部病人均随访12～16月,伤指功能按TAM系统评价。结果:损伤部位在n区的3l例52指病人中优者20指,良28指,可2指,差2指;损伤部位在Ⅲ区的18例32指病人中优者17指,良12指,可3指;总优良率为91.7％。结论:该手术方法为带蒂腱移植,可改善移植腱营养状态,并使移植腱大部分保持在其原有滑行通道内,减少了粘连发生机会。并具有手术操作简单实用,创伤小,疗程短,疗效好等优点。

Objective: This article reports the clinical result of using the flexor digitorum superficialis ( FDS) sliding graft of the injuried finger in reparing flexor digitorum profoundus (FDP) defect. Methods: This kind operation was applied to 84 fingers in 49 patients form October, 1992 to December, 1996. The curative effect of the patients followed up for 12 - 16 months was evaluated by TAM system. Result: Among the 52 fingers in-juried in zone II FDP, 20 fingers were excellent, 28 good, 2 fair and 2 bad, among the 32 fingers injuried in zone III FDP, 17 fingers were excellent, 12 good and 3 fair. The total rate excellent and good results was 91.7% . Conclusions: The operation in this way was a kind of tendon graft with mesolendon pedicle, which improved the nutrient condition of the graft tendon and maintained most of the graft tendon at the original sliding channel, so the opportunity of adhesion was decreased. This kind of operation is simple, applicable, of less scope of wound and of short course of treatment.
