氯氮平、氯丙嗪对血沉及白细胞的影响对照观察

2 Experimental study the effect of clozapine on ESR and WBC

目的探讨氯氮平对血沉（ＥＳＲ）、白细胞（ＷＢＣ）的影响及二者的相关性。方法将入组的精神分裂症病人随机分为氯氮平组（观察组）和氯丙嗪组（对照组），对ＥＳＲ、ＷＢＣ的变化进行为期五周的检测。结果观察组ＥＳＲ异常率７１．８８％，ＷＢＣ异常率５３．１３％；对照组ＥＳＲ异常率６．７％，ＷＢＣ异常率３．３３％，（ＥＳＲχ２＝２６．８３，Ｐ＜０．００５，ＷＢＣχ２＝２０．０５，Ｐ＜０．００５）；观察组ＥＳＲ、ＷＢＣ均于一周内增高至最高值以后逐渐下降，且二者的升降呈高度相关。结论氯氮平可引起ＥＳＲ、ＷＢＣ的改变，可能与个体变态反应有关。

Objective:This work was to explore the effect of clozapine on ESR and WBC,and the relationship between ESR and WBC.Methods:Subjects were randomly assigned either to clozapine group(observation group)and chlorpromazine group(control group)for five weeks obserration perivel,to test the change of ESR and WBC.Results:In observation group,unusual rate of ESR and WBC is 71 88% and 53 13%,respectively.In control group,unusual rate of ESR and WBC is 6 7% and 3 33%,respectively.The two groups have obvious difference.(ESR χ 2=26 83, P <0 005,WBC χ 2=20 05, P <0 005)In observation group,ESR and WBC were increased to the peak within a week,(ESR 46 00±20 41mm/h,WBC 9 99±3 00×10 9/L)then gradotaly decreased,moreover,the level of ESR was significantly correlated with the level of WBC.Conclusion:Clozapine can cause the change of ESR and WBC.It may be related to the individual's allergy.
