低位直肠癌保肛扩大根治手术

Extensive Radical Treatment and Anussaving Operation of Low Rectal Carcinoma

目的 探讨扩大根治手术在低位直肠癌保肛手术中的临床意义。方法 在265例低位直肠癌中选择155例实施保肛手术,对其中62例在扩大根治手术同时完成保肛手术,保肛术式,支撑吻合管经肛门环扎式结肠-直肠(肛管)吻合术。结果 术后随访1～3年,扩大保肛组:3年生存率为73.1％,局部复发率为0,一般保肛组:3年生存率为65.2％,局部复发率为5.16％。排便功能:优占80％,一般占16.5％,差占3.5％。结论 扩大根治手术可有效防止低位直肠癌保肛手术后局部复发,扩大保肛手术适应证,排便功能良好,不增加手术并发症。

Objective To probe into the clinical significance of extensive radical treatment in anussaving operations of low rectal carcinoma. Methods Select 155 cases to carry out anussaving operations in 265 cases of low rectal carcinoma,and 62 cases finished anussaving operations while carrying out extensive radical treatment. Results All the patients were followed-up for 1 to 3 years,in the extensive radical treatment group,3-years survival rate was 73. 1% ,local recurrence rate was 0;in the general group,3-years survival rate was 65. 2%,local recurrence rate was 5. 16%,The function of shitting:excellent rate was 80%, passable rate was 16. 5%, poor rate was 3. 5% ,Conclusions Extensive radical treatment could prevent effectively local recurrence in anussaving postoperation,enlarge indications of anussaving operation,but the function of shitting was well,not increase complications.
