以周围神经病为主要表现的系统性红斑狼疮肌电图分析

EMG ANALYSIS OF SLE WITH PRINCIPAL SYMPTOMS OF PERIPHERAL NEUROPATHY

报告二例以周围神经损害为主要表现的SLE患者,均为女性,分别为25岁和41岁。主要表现为双下肢进行性乏力、关节肿痛。患肢肌力、肌张力降低,腱反射减弱。一例见肌肉萎缩。神经电生理检查示多神经传导速度减慢,波幅降低。一例见纤颤电位和正生理检查与病理特征相符:SLE合并周围神经病同时存在髓鞘脱失和轴索变性。提示生理检查可为SLE合并周围神经病提供客观诊断依据。结合文献复习了SLE合并周围神经病的临床表现、发病机制和病理特征。

In this study,Two female patients with SLE were observed.Their ages arc 25 and 41 years old.The principal symptoms of them were prograssive weakness of lower limbs and swollem and pain of articulation.The principal signs were amyostheni-a,hypomyotonia and weakened tendon reflex.Slowed nervous conductive velocity and decreased amplitude were showed in the neuroelectrophysiologic examination and some Fib and PSW were found in one of them.The results conformed to the pathologic feature that demyelination and axonal degeneration were simultaneous in the peripheral neuropathy of SLE.It showed that the neuroelectrophysiologic examination could provide objective diagnostic basis for the peripheral neuropathy of SLE.Its clinical symptoms,pathogeny and pathologic feature were reviewed with combining literature.
