C型肱骨髁间骨折的手术治疗

Surgical Treatment of Intercondylar Comminuted Fractures of Humerus

目的 探讨肱骨髁间粉碎骨折手术治疗的临床疗效。方法 2 6例 C型骨折 ,双钢板法 14例 ,其中双重建钢板 6例 ,重建钢板加半管状钢板 8例 ,Y形钢板 8例 ,单重建钢板加拉力螺钉 4例。结果 2 6例随访 3个月～ 3年 ,未发现不愈合。按照 Aitken和 Rorabeek标准评定 ,优 12例 ,良 10例 ,可 4例 ,优良率为 84 .6 2 %。结论 早期解剖复位 ,尺骨鹰嘴截骨入路 。

Objective To explore the effects of surgical treatment of the intercondylar comminuted fractures with open reduction and internal fixation.Methods 26 patients with complete clinical data were followed up from 3 months to 3 years.Of the 26,there were 6 cases using double reconstruction plates,8 with “Y”shaped plates,8 with combination of reconstruction plates and half tube-shaped plates,4 combined with reconstruction plates and screws.Results There was no nonunion in all 26 cases.They were evaluated with Aitken and Rorabeek cassebaum rating system.12 patients were rated as excellent,10 as good,4 as fair.Conclusion The degrees of comminution and displacement of fractures were well correlated with clinical results.Early surgical management,with osteotomy of the olecranon,anatomical reduction,rigid fixation and early exercise are the important factors that can improve the final results.
