肺癌并发下呼吸道感染的痰菌及药敏分析

Sputum bacteria and drug-sensitivity analysis of lower respiratory infection with lung cancer

目的 观察肺癌并发下呼吸道感染的菌群分布及药敏情况。方法 5 4例 1 999年 1月～ 2 0 0 2年 9月住院肺癌患者留痰做普通菌培养及真菌培养 ,并做药敏分析。结果 肺癌并发下呼吸道感染病原菌中革兰氏阴性菌感染占 6 6 .7% (4 8/ 72 ) ,革兰氏阳性菌占 1 5 .3% (1 1 / 72 ) ,白色念珠菌占 1 8.1 % (1 3/ 72 )。 91 .7% (4 4 / 4 8) )的革兰氏阴性菌对氨基甙类药物敏感 ,87.5 % (4 2 / 4 8)对头孢哌酮钠 +舒巴坦敏感 ,81 .3% (39/ 4 8)对喹诺酮类敏感。 90 .9% (1 0 / 1 1 )革兰氏阳性菌对万古霉素敏感 ,5 4 .5 % (6 / 1 1 )对头孢哌酮钠 +舒巴坦敏感。结论 肺癌并发下呼吸道感染以革兰氏阴性菌为主 ,对氨基甙类药物、头孢哌酮钠 +舒巴坦、喹诺酮类药物敏感。革兰氏阳性菌对万古霉素、头孢哌酮钠 +舒巴坦敏感。经验选择抗生素时 。

Objective To investigate the bacteria sort and drug sensitivity of lower respiratory infection with lung cancer.Methods Selected all inpatients with lung cncer in tian jin lung disease hospital from 1999,Jun to 2002,Sep,total 54 patients were involved.everyone tool sputum general bacteria culture and fungi culture.and positive culture results were examed the drug sensitivity. Results In the bacteria of lower respiratory infection with lung cancer,Gram negative bacteria was 66.7%(48/72).Gram positive bacteria was 15.3%(11/72),candida albicans was 18.1%(13/72).91.7%(44/48)of Gram negative bacteria were sensitive to aminoglycosides,87.5%(42/48) were sensitive to cefoperazone +sulbactam,81.3%(39/48) wrer sensitive to quinolones.90.9%(10/11)of Gram positive bacteria were sensitive to vancomycin,54.5%(6/11) wrer sensitive to cefoperazone sulbactam.Conclusion Gram negative bacteria is predominant in the bacteria of lower respiratory infection with lung cancer,Gram negative bacteria are sensitive to aminoglycosides,cefoperazone+sulbactam,quinolones.Gram positive bacteria are sensitive to vancomycin,cefoperazone+sulbactam.we suggest slecting mainly anti Gram negative bacteria drug to be empiric antibiotic therapy.
