妊娠期糖尿病孕妇的脂代谢变化

A Study on Lipid Metabolism in Women with Gestational Diabetes

目的了解妊娠期糖尿病(GDM)对孕妇脂代谢的影响,并探讨导致高脂血症发生的相关因素及其临床意义。方法选择我院53例 GDM 孕妇作为观察对象,测定其空腹血脂及脂蛋白与同期糖代谢正常孕妇进行比较,动态观察 GDM 孕妇治疗前后血脂代谢的变化,并且分析了 GDM 伴发妊高征与血脂代谢紊乱的关系。结果 GDM 孕妇存在高脂血症,属Ⅳ型,发病率达34%;GDM 并发高脂血症时妊高征发病明显增加。GDM 经过治疗后血脂仍呈上升趋势。结论 GDM 虽然病程短,仍然对血脂代谢存在一定影响,而且血糖控制后血脂并无改善,今后应注意对 GDM 患者限制碳水化合物的同时进行脂代谢的监测,合理调整饮食中脂肪含量可能将减少脂代谢异常,以减少 GDM时妊高征的发生。

Objective To study lipid and lipoprotein changes in cases with GDM and the cause of hyperlipidemia and its clinical significance.Methods Prospective study was made on 53 GDM cases from 1996,10,to 1997,12,the changes of lipid and lipoprotein were observed during treatment period.Result GDM patients exhibit the TG and VLDL abnormalities,the incidence of hyperlipi- demia in GDM cases was 34%.The incidence of PIH increased in GDM patients with hyperlipidemia. After treatment of GDM the lipid level still increased.Conclusion These results indicate that GDM can influence lipid and lipoprotein metabolism and lipid metabolism abnormalities can not be improved by diet restriction.Our data suggest that altered lipid profiles in women with GDM may re- sult from unadapted proportion of carbohydrate and fat in the diet intake.We should monitor blood lipid and lipoprotein level while maintaining maternal blood glucose level.
