St ThomasⅡ号液对成熟和未成熟豚鼠心肌缺血再灌注损伤的作用

摘要 目的:研究比较StThomasHospitalcardioplegicsolutionⅡ(简称StThomasⅡ号液)对成熟和未成熟豚鼠心肌缺血再灌注损伤的不同作用。方法:建立离体心脏顺行灌注左心做功模型,以StThomasⅡ号液为心停搏液,测定成年(A组)和新生(B组)豚鼠心脏缺血前、后和再灌注时(后)的有关心功能指标,心肌ATP、总腺苷酸和丙二醛(MDA)含量,并行心肌电镜观察。结果:B组缺血再灌注后的心功能恢复,心肌ATP和总腺苷酸的贮备以及心肌超微结构的改善均明显低于或劣于A组,而MDA含量显著高于A组(P<0.05)。结论:StThomasⅡ号液对成熟豚鼠心肌缺血再灌注损伤有良好保护作用,而对未成熟者的作用不满意,这可能与再灌注后新生豚鼠心肌中氧自由基的大量产生及其损害有关。 Objective:To study the different effect of St Thomas Hospital cardioplegic solution Ⅱ on adult(group A)and newborn (group B)guinea pig myocardium. Methods:Isolated antegrade perfused leftside working heart model was established. All hearts were arrested by the cardioplegic solution. Cardiac function, myocardial adenosine phosphate, malodialdehyde (MDA) and ultrastructure were investigated before, after ischemia and after (during) reperfusion. Results:The recovery percent of cardiac funtion and the contente of myocardial adenosine phosphate were significantly lower(P<0.05) and the improvement of myocardial ultrastructure was lower in group B than in group A.Conclusion:The cardioplegic solution can effectively protect mature myocardium, but can not protect immature myocardium from ischemia repefusion injury. It is possibly due to the injuries of oxygen radicals and Ca2+overload, which are severer to immature myocardium than to mature myocardium after(during) ischemia reperfusion.
