肝硬化患者胃运动功能的研究

Study on gastric motility function in patients with liver cirrhosis

目的：研究肝硬化患者的胃运动功能，探讨肝硬化患者的固体食物胃排空与肝功能、胃肠道症状和腹水之间的关系。方法：给51例肝硬化患者及22名健康对照者进食核素标记的固体试餐后，用SPECT测定胃排空。结果：肝硬化患者固体食物胃排空延迟，胃排空与血清胆红素水平呈正相关，与血清白蛋白水平呈负相关；胃排空延迟与胃肠道症状有关，与是否合并腹水无关。结论：肝硬化患者固体胃排空延迟，主要与肝功能的损害程度有关，胃排空延迟与胃肠道症状的产生有一定关系，与合并腹水无关，建议临床上改善肝硬化患者的胃肠症状除保肝外，应重视胃动力异常并给予适当治疗。

Objectives: To study solid gastric emptying by solid meal in patients with liver cirrhosis and investigate the correlations between gastric emptying and liver function, the gastrointestinal symptoms and presence of ascites. Methods: After ingestion of istope labelled solid meal, the gastric emptying was studied by SPECT in 51 patients with liver cirrhosis and 22 normal controls. Results. The gastric emptying in patients with liver cirrhosis was significantly slower than that in normal controls (P<0.01), the patients with liver cirrhosis showed positive correlation between delayed gastric emptying and serum bilirubin levels (r=0.36, P<0.05) and negative correlation between delayed gastric emptying and serum albumin levels (r=-0.29, P<0.05), also a correlation between delayed gastric emptying and gastrointestinal symptom score (P<0.05),whereas where was no correlation between delayed gastric emptying and presence of ascites.Conclusions: gastric emptying by solid meal in cirrhotic patients was significantly slower than that in normal controls which was caused by liver dysfunction. It is suggested that treatment should be aimed at improving the gastric motility and liver function.
