以充气背心为主体的新型双泵复苏术血流动力学研究(2)

摘要 以充气背心为主体的新型双泵复苏术(VDVEST-CPR)能改善冠脉有效灌注压(CPP),观测其复苏效果随时间变化.以IBMPC386为控制主机的多功能复苏装置,在8条犬心室颤动30s后随机地实施充气背心复苏术(VEST-CPR)及VDVEST-CPR30min,每隔5min观测一次,自身对照设计.结果:整个实验中,VDVEST-CPR较VEST-CPR显著升高主动脉和右房的舒张压(P<0.01).主动脉舒张压升高程度较右房舒张压大,CPP显著性升高(P<0.01).虽然随复苏时间延长,复苏效果有所下降,但是VDVEST-CPR始终优于VEST-CPR.VDVEST-CPR30min的复苏中,血流动力学效果优于VEST-CPR. It has been reported that a novel CPR named as VDVEST-CPR which was inflation vest assisted with external (CPP).The between hemodynamic effect of VDVEST-CPR and resuscitation time was studied.Methods:After 30s of ventricular fibrillation both VDVEST-CPR and VEST-CPR were executed in random order on each of 8 d and alternated in 90 s interval.Two CPRs were repeated per 5 min for 30 min.statistical analysis used by paired-t test and linear regression.Results:During the experiment of 30 min the diastolic pressure of aorta (Pao) and right atrium (Pra) in VDVEST-CPRR were significantly higher than in VEST-CPR (P< 0.01), but the increment of diastolic Pao was greater than that of diastolic Pra,so that CPP were improved significantly in VDVEST-CPR (P<0.01).Although CPP was decrease gradually,CPP of VDVEST-CPR was greater than VEST-CPR.Conclusion:The hemodynamic effect of VDVEST-CPR was superior to VEST-CPR in the canine experiment.
