选择性射频热凝治疗三叉神经痛

Selective percutaneous radiofre- quency treatment for trigeminal neuralgia (A study of 147 cases)

目的:对147例采用选择性射频热凝治疗三叉神经痛病例疗效进行分析讨论.方法:仰卧位,Hartel前入路穿刺法,局部浸润麻醉,刺中三叉神经节后以温控射频热凝对靶点毁损.作者对此项技术作了部分改进.结果:疗效优良112例,良好28例,无变化7例,总有效率95.2%.结论:认为改进后的方法简便、安全、有效、对老年人更为适宜.每次毁损参数以温度60～75℃,热凝时间2～4min为好.

Selective percutaneous radiofrequency for trigem-inal neuralgia was performed in 147 patients. Of them 77 were males and 70 females. Their age ranged from 20 to 74 years (average 51 years). A special designed stereotactic instrument was used for this procedure. This instrument can approach the target of the foramen ovale accurately and rapidly. This kind of treatment is suitable foraged and weak patients who can not tolerate conventional surgery. This procedure, which had been used in treating trigeminal neuralgia by the authors for more than 10 years,and proved to-be safe, less painful and tolerable by the patients. The results were excellent in 112 cases (82% ), improved in 28 cases (14 % ) and no change in 7 cases ( 4. 0 % ) . No serious complication and no mortality in this group. The indications, and techniques of this procedure were discussed in this paper.
