血液细胞化学和生物化学参数对尘肺亚临床诊断的研究

摘要 目的:尘肺是一种严重危害劳动生产力的职业病。对尘肺的亚临诊断国内外学者从血液生化指标和肺泡灌液指标做了许多研究,但从血液细胞化学和生物化学参数对尘肺亚临床诊断尚未见有报道。方法:通过测定接尘工人、尘肺病人和健康对照人群血液细胞化学和生物化学参数,使用Bayes准则逐步判别分析,对尘肺进行亚临床诊断。结果:结果显示,血液中性粒细胞碱性磷酸酶,糖原,脂肪和过氧化物酶及红细胞谷胱甘肽和乳酸脱氢酶六项指标,对对照组,现接生组,远接尘组,矽肺组和煤工尘肺组有一定的判别效果,总判别符合率为65.52%,对其各组的判别复符合率分别是90.32%,71.88%,45.46%.65.00%,40.00%。结论:结合判别效果和符合率,表明此六项指标均显示对生肺亚临床诊断有良好的判别能力,具有一定的尘肺亚临床诊断价值。 Objectives: Pneumoconiosis is an occupational disease that hazard the health of labour force. For the diagnosed subclinical pneumoconiosis have had many studies through the biochemical indexes of blood and bronchial alveolar of lung fluid (BALF) over the world. But in the present there are a few of reports for the diagnosed subclinical pneumoconiosis through the parameters of cytochemical and cytobiochemical of blood. Methods: We measured the blood parameters of oytochemical and cytobiochemical of expoure dust,pneumoconiosis and control. Using the method of Bayes principle stepwise discrimination. we studied the value of subclinical diagnosed for pneumoconiosis through the parameters of cytochemial and cytobiochemical of blood. Results: Results showed that the six indexes of alkaline phosphoesterase. glycogen , fat . peroxidase of neutrogranulocytes and glutathione. lactic acid hydrogenlyase of erythrocytes have ansure discriminate ability for the control, present exposure to dust. past exposure to dust, siliccsis and coal workers pneumoconiosis. The coincident rate of total discriminate back substitution were 65. 52%and other groups respectively were 90. 32%. 71. 88%. 45. 46%. 65. 00%. 40. 00%. Conclusions: Combined the discriminate ability with the coincident rate of discriminate back substitution indicated that six indexes were sensible discriminate effects for the phase of subclinical pneumoconiosis and had an assure diagnosed value for the phase of subclinical pneumoconiosis.
