AO苜蓿叶形支撑钢板治疗Pilon骨折

Surgical Treatment of Tibial Pilon Fractures with Cloverleaf Buttress Plates

目的 总结使用AO苜蓿叶形钢板治疗Pilon骨折的经验。方法 分析我科自 1 996年 3月～ 2 0 0 1年 6月用该方法治疗的 2 3例病人的治疗效果 :按Ruedi Allgower分型 :其中Ⅱ型 1 8例 ,Ⅲ型 5例 ,均采用切开复位 ,AO苜蓿叶形钢板内固定术。结果 治疗后 ,临床疗效优良率 82 .6 % ,其中Ⅱ型达 88.8% ,Ⅲ型达 60 %。结论 AO苜蓿叶形钢板治疗Pilon骨折 ,具有骨折稳定性可靠 ,有利于骨折愈合和早期关节活动的优点 。

Objective To sum up the clinical experience for tibial pilon fractures treated by open reduction and internal fixation with the cloverleaf buttress plates.Methods From March 1996 to June 2001,23 patients suffered with pilon fracture were treated with this method.Fractures were classified as Type Ⅱ(18 cases) and Type Ⅲ(5 cases) according to the criterion difined by Ruedi and Allgower.All cases were treated by open reduction and internal fixation with the cloverleaf buttress plates.Resutls Postoperatively,82.6 % of the patients had satisfactory limb alignment,88.8 % of Type Ⅱ and 60 % of type Ⅲ fractures had good or excellent results.Conclusion Followed AO principles,open reduction and internal fixation with the cloverleaf buttress plate to treat tibial pilon fractures can provide stable fixation,rapid bone union and early joint exercise.However we should develop alternative method to treat the type Ⅲ fractures.
