背景:世界卫生组织/国际防痨和肺病联合会(WHO/IUATLD)的全球抗结核病药耐药性检测规程的质量保证。目的:在WHO/IUATLD超国家参比试验室(SRL)网内部进行药敏试验熟练程度测试。设计:网内16家实验室用同样的培养物清单进行测试.每组均含有20份结核分支杆菌临床分离物,其中既有药敏培养物,也有耐药培养物,测试项目是链霉素、异烟肼、利福平和乙胺丁醇的耐药性。药敏试验包括比例法、绝对浓度法和耐药性比率法及其改良法,包括BACTEC 460放射测量法。结果:第一轮熟练程度测试表明,在SRL网内药敏试验的特异性显著高于其敏感性。异烟肼和利福平测试在各实验室之间高度一致,但链霉素和乙胺丁醇的结果不甚一致。结论:有两种抗结核病药,即异烟肼和利福平的药敏测试的各步骤在SRL网内是高度可靠的,它们可用来确定多药耐药性结核病。链霉素和乙胺丁醇的药敏试验的步骤尚有待标准化。
