椎—基底动脉供血不足的BAEP与TCD对比性分析

COMPARATIVE STUDY OF BAEP AND TCD IN PATIENTS WITH VERTEBROBASILAR BLOOD INSUFFICIENCY

对42例发作期椎—基底动脉供血不足(VBI)患者进行了脑干听觉诱发电位(BAEP)检查,其中并作经颇多普勒(TCD)检查23例,其异常率分别为88.1％、82.6％。本病临床上主观症状多,客观诊断依据不足,神经系统体查及CT、MRI等检查多不能获得满意结果。BAEP能敏感地记录到缺血引起的脑干神经细胞电生理改变,TCD则能直接反映椎—基底动脉血流动力学的改变。尤其在发作期及时地检查对VBI早期诊断、早期治疗具有重要的指导意义。本文并对4例TCD正常,BAEP异常的病例,结合病因进行了初步分析。

Fourty-two patients with vertebrobasilar blood insufficiency (VBI) in attack stage were studied by brainstem auditory evoked potential (BAEP),and twenty-three of them were also studied by transcranial doppler (TCD).The abnormal rates were 88.1%,82.6%,respectively.There are a lot of subjective symptoms in this disease but the objective diagnostic bases weren't enough.Nervous system test CT,and MRI.etc.can't get the satisfactory result.But BAEP can sensitively record the electrophysiological changes of the brainstem nervous cells.TCD can directly explore the hemodynamics change of the vertebro-basilar blood flow.There are very important instructive effect for the early diagnosis and treatment of VBI,especially in stage of attack.
