婴儿肝炎综合征病因探讨

Study on Viral Cause of Hepatitis Syndrome

我科于1983年1月～1993年12月，对诊断婴儿（包括新生儿）肝炎综合征的病儿92例，进行尿病毒检测70例，结果人类巨细胞病毒阳性44例，占63％，乙肝病毒阳性5例，单纯疱疹病毒及类冠状病毒各1例，总病毒检出率为72．9％。提示病毒感染，特别是人类巨细胞病毒感染是婴儿肝炎综合征的重要病因。经住院治疗4周。黄疸无明显减退者，应用单光子同位素断层扫描（SPECT）进行肝胆动态显像检查13例，结果11例为胆道闭锁，2例为肝内毛细胆管炎。此13例中尿人类巨细胞病毒（HCMV）阳性11例，提示肝胆管闭锁与人类巨细胞感染有关。

Urine viruses were detected in 70 eases out of 92 in patient infants (including neonates) with hepatitis syndrome. The result showed human cytomegalovirus positive in 44 cases, HBV in 5 cases, herpes virus in 1 case and coranalike the in 1 case. The HCMV positive rate was 63%, and total virus positive rate was 72.9%.It indicated HCMV was the main cause of infantal hepatitis syndrome.If after 4 weeks of systemic treatment the jaundice did not subside, we used SPECT to detect the causes. The results showed biliary tract obstruction in 11 cases and intrahepatic cholangiolitis in 2 cases. It indicated that hepatic and biliary tract obstruction of infantal hepatitis syndrome associated with HCMV infection.
