异丙酚配伍麻黄碱和昂丹司琼用于无痛人流术30例

Anesthetic Effect of Propofol Intermixed with Ephedrine and Ondansetron in 30 Cases of Induced Abortion

目的 :观察异丙酚伍用麻黄碱、昂丹司琼用于人工流产的麻醉效果。方法 :90例早孕妇女 ,随机分为A、B、C三组 ,每组各 3 0例 ,术前A组给予 1%异丙酚 2 .2mg·kg 1 ,iv ,B组给予 1%异丙酚 2 .2mg·kg 1 和麻黄碱 1.65× 10 3mg·kg 1 ,iv ;C组给予 1%异丙酚 2 .2mg·kg 1 +麻黄碱 1.65× 10 3mg·kg 1 +昂丹司琼 4mg ,iv。记录用药前、术中、术后 1,3min的平均动脉压 (MAP)、心率 (HR)、血氧饱和度 (SPO2 ) ,同时观察不良反应、唤醒时间和离院时间。结果 :A组患者用药后MAP、HR降低 ,与用药前差异有显著性 (P <0 .0 5 ) ,B、C组患者用药前后循环稳定。组间比较 ,用药后B、C组MAP高于A组 (P <0 .0 5 )。三组患者术中SPO2 、唤醒时间和离院时间差异无显著性。A、B组各有 4例 ( 13 .3 3 % )患者术后发生恶心呕吐。结论 :异丙酚联合低浓度麻黄碱用于人工流产术的麻醉安全有效 ,循环平稳 ,再联合昂丹司琼可降低术后恶心。

Objective:To observe the anesthetic effect of propofol intermixed with ephedrine and ondansetron used in induced abortion. Methods:Ninety pregnant women were randomised into A、B、C group evenly(n=30). Before operation, Group A, B, C were given 1% propofol 2.2 mg·kg -1, 1% propofol 2.2 mg·kg -1+ ephedrine 1.65×10 -3 mg·kg -1, and 1%propofol 2.2 mg·kg -1+ephedrine 1.65×10 -3 mg·kg -1+ ondansetron hydrochloride 4 mg respectively by intravenous injection. MAP、HR、SPO 2 were recorded at the time before injection, during operation, 1 min after operation, and 3min after operation. Complications, evoking time and leaving time were also recorded. Results:MAP, HR of group A fell significantly after injection propofol (P<0.05). Blood circulation kept steady in group B or C. After injection, MAP of both group B and group C were significantly higher than that of group A (P<0.05). The difference of SPO 2、evoking time、leaving time between A and B or C group were not significant. Four patients (13.33%) of group A and four patients of group B felt nausea or vomit after the operation. Conclusion:Propofol in compatible use with low concentration ephedrine and ondansetron in anesthesia for induced abortion patient is safe and effective, which reduced the adverse reaction.
