HMB－45单克隆抗体在黑素瘤免疫组织化学诊断中的应用

APPLICATION OF HMB-45 MONOCLONAL ANTIBODY IN DIAGNOSIS OF MELANOMA

目的研究ＨＭＢ－４５单克隆抗体免疫组织化学在诊断黑素瘤中的应用价值。方法采用抗ＨＭＢ－４５单克隆抗体ＡＢＣ法检测５例正常皮肤、１０例皮内痣，６７例黑素瘤和６４例其他肿瘤的ＨＭＢ－４５表达情况。结果ＨＭＢ－４５表达仅见于黑素瘤，阳性率为９４．０％（６３／６７）。其中原发性（１００％，１２／１２）和转移性（９２．７％，５１／５５），无色素性（９３．８％，３０／３２）和色素性（９４．８％，３３／３５），梭形细胞型（８８．９％，８／９）、多边形细胞型（９４．６％，３５／３７）、立方形细胞型（９４．１％，１６／１７）和气球样细胞型（１００％，４／４）间表达的差异无显著意义（Ｐ＞０．０５）。ＨＭＢ－４５表达与Ｓ－１００蛋白和ＮＳＥ相比特异性较高。结论ＨＭＢ－４５可能成为诊断黑素瘤极有用的标志。

PURPOSE To study value of HMB-45 monoclonal antibody in the diagnosis of melanoma.METHODS Immunohistochemical detection of HMB-45 monoclonal antibody was investigated in 5 cases of normal skin, 10 of intradermal nevus, 67 of melanoma and 64 of non-melanoma.RESULTS The expression of HMB-45 was only found in melanomas (94.0%, 63/67). In expression of HMB-45,there was no significant difference (P>0.05) between primary (100%, 12/12) and metastatic (92.7% , 51/55) melanoma, between non-pigmented (93.8%, 30/32 ) and pigmented (94.1%, 16/17 ), and between spindle (88.9%, 8/9),polygonal (94.6% , 35/37 ),cuboidal (94.1%, 16/17) balloon cell (100%, 4/4). The expression of HMB-45 in melanoma was more specific than that of S-100 protein and NSE.CONCLUSIONS The results suggest that HMB-45 may be a useful marker in the diagnosis of melanoma.
