摘要
目的:探讨老年慢性阻塞性肺疾病(chronicobstructivepulmonarydis-ease,COPD)患者血浆白细胞介素18(interleukin-18,IL-18)、IL-16,IL-4与干扰素-γ水平的相关性。方法:老年COPD患者共73例(重度21例,中度21例,轻度31例),正常对照31例。采用双抗体夹心酶联免疫吸附实验。检测其血浆IL-18,IL-16,IL-4,干扰素-γ水平。结果:①COPD患者(重、中、轻)急性期血浆IL-18水平(ng/L)分别为409.35±80.30,243.08±81.16,237.29±89.23,均明显高于对照组(139.40±43.74),差异有显著性意义(t=3.91,3.03,2.88,P均<0.01);IL-4水平分别为1.75±0.73,2.03±1.23,2.01±0.99,均明显低于对照组(3.08±1.25),差异有显著性意义(t=-4.78,-2.99,-3.68,P均<0.01);重度、中度组IL-16水平犤(270.60±90.78),(235.48±90.17)犦、干扰素-γ水平(ng/L)犤(11.05±4.55),(2.11±1.33)犦分别显著高于对照组犤(126.73±57.64),(1.03±0.51)犦,差异有显著性意义(t=6.95,6.17,10.05,3.71,P均<0.01)。②治疗后,各组(重、中、轻)缓解期血浆IL-18,IL-16、干扰素-γ水平均明显减低,差异有显著性意义(t=2.03~10.29,P均<0.01);重、轻度患者IL-4水平明显升高,差异有显著性意义(t=-5.86,-3.52,P均<0.01)。③急性期血浆中IL-18,IL-16。
AIM:To investigate the correlations of plasma levels of interleukin 18(IL 18),IL 16 and IL 4 with interferon γ(IFN γ) in elderly patients with chronic obstructive pulmonary disease(COPD). METHODS:A total of 73 elderly patients with COPD(21 severe,21 moderate and 31 mild) and 31 healthy controls were involved.The levels of IL 18,IL 16,IL 4 and IFN γwere detected with the double antibody sandwich enzyme linked immunoadsordent assay(ELISA). RESULTS:①At acute attack stage, the plasma levels of IL 18 of patients with severe,moderate and mild COPD were(409.35±80.30),(243.08±81.16) and(237.29±89.23) ng/L,respectively,all significantly higher than that in the control group[(139.40±43.74) ng/L](t=3.91,3.03,2.88,P< 0.01); and the IL 4 levels[(1.75±0.73),(2.03±1.23) and(2.01±0.99) ng/L,respectively] were significantly lower than that in the control group[(3.08±1.25) ng/L](t=-4.78,-2.99,-3.68,P< 0.01);The IL 16 levels[(270.60±90.78) and(235.48±90.17) ng/L] and the IFN γlevels[(11.05±4.55) and(2.11±1.33) ng/L] of the severe and moderate patients were significantly higher than those in the control group[(126.73±57.64) and(1.03±0.51) ng/L](t=6.95,6.17,10.05,3.71,P< 0.01).②At the remission stage after treatment,the plasma levels of IL 18,IL 16 and IFN γof the severe,moderate and mild patients were all obviously decreased with significantly differences(t=2.03-10.29,P< 0.01);and the IL 4 levels of severe and mild patients were increased obviously with significant differences(t=-5.86,-3.52,P< 0.01).③At acute attack stage of COPD,there were significant correlations among the plasma levels of IL 18,IL 16 and IFN γ(r=0.256-0.918,P< 0.05). CONCLUSION:COPD is characterized by a predominant expression of Th1 type cytokines in elderly patients.The elevated IL 18 and IL 16 levels may be associated with the increased secretion of IL 18 and IL 16 from mast cells.Monitoring the dynamic changes of plasma levels of IL 18 and IL 16 during the treatment of COPD may reflect the inflammatory conditions in the airways.
