神经衰弱二重辨证施治在开放式心理病房的应用

Application of double-syndrome differentiation treatment on neurasthenia in open wards

目的:探讨二重辨证施治对神经衰弱的疗效,及其在开放式心理病房的适应情况。方法:将65例神经衰弱患者按随机的原则在开放式心理病房分为研究组(33例)和对照组(32例)。研究组用二重辨证施治,对照组用认知疗法结合阿普唑仑、氟西汀等。研究时间共5个月,前2个月为积极治疗期,后3个月为自由治疗期。采用临床疗效评定标准,90项症状自评量表(SCL-90)。结果:研究组和对照组,在积极治疗期末分别痊愈33例和25例,两组疗效差异有显著性意义(χ2=5.97,P<0.05);在自由治疗期末分别痊愈33例和19例,两组疗效差异有极显著性意义(χ2=14.31,P<0.005)。研究组和对照组的SCL-90总分,在治疗前分别为(241.79±48.62)和(244.43±54.12),两组差异无显著性意义(t=0.201,P>0.05);在积极治疗期末分别为(109.21±8.447)和(125.94±17.56),两组差异有极显著性意义(t=4.917,P<0.001);在自由治疗期末分别为(106.67±9.10)和(149.47±55.00),两组差异也有极显著性意义(t=4.409,P<0.001)。结论:二重辨证施治对神经衰弱有普遍卓越的痊愈功效和痊愈巩固功效,开放式心理病房可作为神经衰弱二重辨证施治的医疗平台。

AIM:To study the effect of double syndrome differentiation treatment on neurasthenia and its suitability in open wards. METHODS:Sixty five patients with neurasthenia were randomly divided into study group(n=33) and control group(n=32) in open wards.Patients in the study group received the double syndrome differentiation treatment, while patients in the control group were treated with the cognitive therapy combined with alprazolam and fluoxetine,etc.The treatment lasted for 5 months,including two month active therapy and the next three month free therapy.Clinical scale,including symptom checklist(SCL 90),was used. RESULTS:At the end of the active therapy,there were 33 cases and 25 cases who recovered in the study and control groups,respectively,and the difference of curative effect was significant(χ2=5.97,P< 0.025);and there were very significant differences during the free treatment period between the study group(33 cases) and control group(19 cases)(χ2=14.31, P< 0.005).Before therapy,the total scores of SCL 90 in the study group and control group were 241.79±48.62 and 244.43±54.12 respectively, and there was no significant difference(t=0.201,P >0.5);while the differences were very significant at the end of the active treatment period (109.21±8.447 and 125.94±17.56,t=4.917,P< 0.001) and the end of the free treatment period(106.67±9.10 and 149.47±55.00,t=4.409,P< 0.001). CONCLUSION:Double syndrome differentiation treatment has a satisfactory curative effect and prognosis on neurasthenia,and open wards can be taken as its medical platform.
